Relationship between lymphovascular invasion and features of papillary thyroid cancer

Thyroid cancer is occurs when abnormal cells begin to grow in a thyroid gland. It is the most frequent cancer among endocrine gland cancers. Most thyroid cancers are curable, because the cancer is usually found in early stage and the treatment works well. Although it is treated, thyroid cancer may recur after many years. There are many factors, which affect the recurrence or metastases of thyroid cancer.

Adverse histopathological properties for thyroid cancer survival are described in several studies. In the current study, we investigated the relationship of lymphovascular invasion (LVI) presence with pathological and clinical features and outcomes of thyroid papillary cancer.

We studied 678 papillary thyroid cancer patients between 2010 to 2015. LVI was confirmed in 63 (9.3%) patients. Lymph node metastasis was significantly more common in the group with LVI, and the risk for recurrence or metastases was 30.61 times higher.

The relationship of LVI presence and adverse outcomes of thyroid cancer should steer clinicians to close follow-up and re-evaluation of the patient. LVI presence may be a decision-making factor in adjuvant therapy strategies, and may be added as a parameter in staging systems.

Tiroid Papiller Kanserinin Klinikopatolojik Özelliklerinin  Lenfovasküler İnvazyon ile İlişkisi

Tiroid kanseri tiroid bezi içerisindeki anormol kontrolsüz büyüyen hücreler sonucu gelişen bir hastalıktır. Endokrin kanserler içerisinde en sık görülen kanser türüdür. Enken dönemde teşhis edildiği için tiroid kanserlerinin çoğunluğu tedavi edilebilir ve tedavi süreci genellikle başarılıdır. Bununla beraber tiroid kanseri yıllar sonra bile tekrar edebilir. Hastalığın tekrar etmesinde veya metastaz yapmasında birçok faktör rol oynar.

Birçok çalışmada tiroid kanser seyrini tarifleyen olumsuz histopatolojik faktör tarif edilmiştir. Mevcut çalışmamızda tiroid kanserinde klinik özellikler ve  histopatolojik bulguların lenfovasküler invazyon varlığı (LVI) ile ilişkisi değerlendirildi.

Bu çalışmaya 2010-2015 yılları arasında ameliyat edilen 678 tiroid kanseri hastası dahil edildi. Hastaların  63’ünde (%9.3) LVI (+) bulunurken 615 (%90.7) hastada LVI (-) bulundu. LVI (+) hastalarda lenf bezi metastazı anlamlı olarak yüksek bulundu ve bu risk 30.61 kat fazla görüldü.

Tiroid kanserinde  LVI varlığı klinisyenleri hastalığın kötü gidişatı için uyarıcı bir faktör olabileceği konusunda alerme etmeli ve hastayı yakın takip ve tekrar değerlendirmede rol oynayabilmelidir. Tiroid kanserinde LVI varlığı tiroid kanser hastası tedavisinde karar vermede bir etmen olabileceği ve evreleme sistemlerinde yer alabileceği görüşündeyiz.
