Cytokines may indicate the lack of oxygen severity in newborns and predict its consequences

Can we timely prevent the consequences?

Perinatal hypoxia-ischemia is an important pathological event in everyday neonatal practice. Numerous studies have found that a newborn reacts on perinatal hypoxic events with its whole body. Short-term hypoxia effects can be acute or chronic, of which the most serious and severe consequences are due to hypoxic brain injury. Cytokine paradigm was revolutionary theoretical explanation of the pathophysiological mechanisms in these complex events. Despite numerous studies with controlled animal models, as well as on specific human pathological conditions, the occurrence and role of certain cytokines remain unclear.

It is believed that cytokines have a similar role in both blood and cerebrospinal fluid of hypoxic newborn. However, there are very few studies that examine the occurrence of pro-inflammatory cytokines in the two important systemic compartments simultaneously. With this prospective cohort study, which included children with the selected pathological state over several years, an attempt was made in order to determine the occurrence of the most highly investigated pro-inflammatory cytokines (IL-6, IL-1β, TNF-α) and one immunomodulatory rarely studied cytokine (IL-18), simultaneously, in two systemic compartments of hypoxic newborns.

The main question of our study was: Do studied cytokines appear in detectable concentrations in the body of a hypoxic newborn in the acute phase (the first four days of age)? In addition,  do cytokines appear in both systemic compartments (blood and cerebrospinal fluid) uniformly or perhaps differently? In order to answer these questions, the average values of the concentration of certain cytokines were compared with the degree of acute brain injury (encephalopathies) as well as with the subsequent neurological outcome (i.e., motor injury at the age of 12 months).

Our research has shown that in a period of 3-96 hours after the hypoxic insult, the concentrations of certain cytokines were measurable in the blood and in the cerebrospinal fluid (IL-6, TNF-α), some were non-detectable in both compartments (IL-1β) and one was registered only in the blood of hypoxic newborns (IL-18). It was found that concentrations were significantly elevated in the blood. Although infants showed various degrees of post-asphyxial encephalopathy, no significant correlation between concentration of specific cytokines and the degree of encephalopathy was found. Unlike acute condition, for some cytokines we have found a correlation between increased concentrations and subsequent neurological deficiency.

This study demonstrates that cytokines appear at different concentrations in various systemic compartments, even though a newborn has a global hypoxic-ischemic disorder. Thereby, the occurrence of higher serum concentrations than in the cerebrospinal fluid, could be a reflection of more active pathophysiological events. Possible implication of this study is that blood circulatory system reacts more strongly to hypoxia, thereby possibly protecting the central nervous system. Also, it is possible that disturbance in the complex cytokine network is more damaging in the long term, than in the short term. The results may be of limited significance due to the small sample of patients (pathology is delicate and rare), but also because of the time of sampling, since dynamics of cytokines depends on it. There is a need for more similar studies that will observe and describe this problem.

Da li razine citokina mogu ukazati na težinu nedostatka kisika kod novorođenčadi i predvijeti njegove posljedice?

Perinatalna asfiksija/hipoksija važan je patološki događaj u svakodnevnoj neonatološkoj praksi. Brojna istraživanja utvrdila su da na hipoksijsku noksu novorođenče reagira cijelim tijelom. Efekti kratkotrajne hipoksijske nokse mogu biti akutni i kronični, od kojih su najteži i najozbiljniji posljedica hipoksične ozljede mozga. Citokinska paradigma bila je revolucionarno teorijsko objašnjenje patofizioloških mehanizama u tim složenim događajima. Unatoč brojnim studijama kako na kontroliranim animalnim modelima, tako i na određenim humanim patološkim stanjima, pojava i uloga pojedinih citokina i dalje ostaje nerazjašnjena.

Vjeruje se da citokini imaju sličnu ulogu i u krvi i u cerebrospinalnom likvoru hipoksičnog novorođenčeta. Ipak, vrlo je malo studija koje ispituju pojavu proupalnih citokina u dva važna tjelesna odjeljka istovremeno. Ovom prospektivnom kohortnom studijom, koja ja uključivala djecu s odabranim patološkim stanjem kroz nekoliko godina, pokušalo se odrediti pojava najistraživanijih proupalnih (IL-6, IL-1β, TNF-α) i jednog imunomodulatornog, rijetko istraživanog citokina (IL-18), istovremeno u dva tjelesna odjeljka hipoksičnog novorođenčeta.

Glavno pitanje naše studije bilo je: Pojavljuju li se istraživani citokini u zamjetnim koncentracijama u tijelu hipoksičnog novorođenčeta u akutnoj fazi hipoksije (prva četiri dana starosti)? Dodatno, pojavljuju li se citokini u oba tjelesna odjeljka (krv i likvor) jednakomjerno ili možda drugačije? Da bismo odgovorili na ova pitanja, prosječne vrijednosti koncentracija pojedinih citokina komparirane su sa stupnjevima akutne moždane ozljede (encefalopatije), kao i s kasnijim neurološkim ishodom (tj. motoričkom ozljedom u dobi od 12 mjeseci).

Naše istraživanje pokazalo je da su, u razdoblju od 3-96 sati nakon hipoksičnog inzulta, koncentracije određenih citokina bile mjerljive i u krvi i u likvoru (IL-6, TNF-α), pojedinih nemjerljive u oba odjeljka (IL-1β), dok je jedan registriran samo u krvi hipoksičnog novorođenčeta (IL-18). Pri čemu je nađeno da su koncentracije značajnije povišene u krvi. Iako su novorođenčadi pokazivala različite stupnjeve postasfiktične encefalopatije, nije ustanovljena značajnija povezanost koncentracija pojedinih citokina i stupnja encefalopatije. Za razliku od akutnog stanja, za neke citokine našli smo povezanost povišenih koncentracija i kasnije pojave neurološke deficijencije.

Ovo istraživanje pokazuje da se citokini pojavljuju u različitim koncentracijama u različitim tjelesnim odjeljcima, iako novorođenče ima globalni hipoksično-ishemični poremećaj. Pri tome, pojava viših koncentracija u serumu nego u likvoru moguće odražava mjesto aktivnijih patofizioloških zbivanja.
Moguća implikacija ovog istraživanja je da krvni cirkulacijski sustav na hipoksiju reagira snažnije, pri tome možda štiteći središnji živčani sustav. Također, moguće je da poremećaj složene mreže citokina više šteti dugoročno nego kratkoročno. Rezultati mogu biti od limitiranog značaja, zbog malog uzorka pacijenata (patologija je delikatna i rijetka), ali i zbog vremenskih perioda uzorkovanja, obzirom da dinamika citokina ovisi o trenutku uzimanja uzorka. Potrebno je više sličnih istraživanja, koja bi opservirala i opisala ovaj problem.
